Referring Providers


Download the spreadsheet, complete your referring provider information,
and reply to your welcome email with the completed file.

Send Supporting Documents


Reply to your welcome email with any supporting documents that apply to your setup:

  • Government-issued photo ID
  • Selfie holding your photo ID
  • DEA documentation, if applicable
  • Voided check, if using Auto Debit

What Happens Next?


Once we receive your information, our team will review your submission
and contact you if anything else is needed. From there, we’ll begin configuring
your MedPointe environment and guide you through the remaining implementation steps.
